The Intake Procedures are as Follows:

Phone Interview:

The person seeking treatment shall call the admissions office, at 706-453-7929, and complete a phone interview. A family member, relative, or another person with knowledge of the person can call if the person is unable to make the call due to incarceration, hospitalization, or other reasons. (Admissions will make contact with the person seeking treatment at a later time)


Medical Clearance

The person seeking treatment must have the following required medical clearances completed:

A nursing assessment is required to be completed on the day of the intake appointment and faxed to admissions.

A statement by a licensed medical provider that medically supervised detoxification is not required.

A Rapid Plasma Reagin (RPR) test. Test for syphilis.

A Mantoux tuberculin skin test (TST) or TB blood test. Test for Tuberculosis.

Financial:
 

The person seeking treatment, a family member, or another person will make financial arrangements for the cost of the program with admissions. 

Appointment:

Once all admission criteria are met, an appointment (location, date and time) will be set.  Failure to be on time may result in rescheduling admission at a later date. For a free and confidential consultation, contact our admissions team at 706-453-7929.
